HOW DO I MAKE A SAILOR COLLAR!!!!!!!!

I'm really not sure how to make a sailor collar, it's for my Rin kagamine cosplay.
Do i have to make the shirt too, or can I just stick the collar on to an already made shirt?
PLEASE HELP!!!

It would be possible to attach a collar to a ready made shirt if you adjust the neck opening.
